This form is for Non Residential Passport (NRP) or Right of Choice (ROC) Players only (NOT FHA members) NRP: To help expedite the sign in process at tryouts please complete and submit the following information. Your electronic NRP passport must be on file with the FHA and your tryout fee (cash only) will be collected at your skate. ROC: Please complete this form for review.
